Care Services: Acute and Transitional Care

After a hospital stay, many patients cannot return home at once. In this case, two weeks of acute and transitional care can be prescribed to help them adjust and get ready for their time at home.

Together with our patients, we make treatment plans tailored to their personal needs and work intensively to train them in the best possible way for their after-hospital life.

This service is available to the patients who are residents of the canton of Thurgau.
